  • To collaborate with us, simply refer your patient or student using our referral portal. We will then:

    1. Contact the family you referred within one business day

    2. Update you on the status of your referral

    3. Update you on progress every 4 sessions (assuming the family does not object)

    We do not need you to sign any contracts with Fort Health. We do not charge you, your practice, your school or your district.
